Pregnancy/Delivery Course: The pregnancy was complicated by Hep C, bipolar, Unknown GBBS and polysubstance drug use. UDS during pregnancy: + cocaine, amphetamines, opioids,THC. Most recent drug screen +opioids. Currently on Subutex. Prenatal ultrasound revealed normal anatomy. Prenatal care was late. Mother received Penicillin G x 2 doses. Membranes ruptured on 2/13/2020 at 1700 by SROM. The delivery was complicated by thick meconium. NICU attended infant required deep suction and BBO2. Apgar scores Newborn Assessment: 1 Minute:

Review of Systems Constitutional: Negative for decreased responsiveness, fever and irritability. HENT: Negative for congestion, rhinorrhea and trouble swallowing. Eyes: Negative for discharge and redness. Respiratory: Negative for apnea, cough, choking, wheezing and stridor. Cardiovascular: Negative for cyanosis. Gastrointestinal: Negative for abdominal distention, blood in stool, diarrhea and vomiting. Genitourinary: Negative for decreased urine volume and scrotal swelling.
Musculoskeletal: Negative for extremity weakness. Skin: Negative for color change, pallor and rash. Neurological: Negative for seizures and facial asymmetry.
