A copy of the completed User questionnaire is included in Appendix B. VERTEX did not receive land title records from the User. The User did not provide information relative to AULs or environmental liens at the subject property. The User identified that a prior Phase I ESA was completed by VERTEX for the subject property in 2021, which is discussed in Section 5.3 of this report. No other responsive information regarding the subject property was provided by the User.
